- Respondents should not submit any proposals through their BizNet accounts. However, confirmation that an account has been created in the Connecticut Department of Administrative Services (“DAS”) Business Network (“BizNet”) system showing that all required forms have been completed and uploaded is required. The required forms may include, but may not be limited to: Ethic Affidavits and Certifications Form 1 (Gift and Campaign Contribution); Form 5 (Consulting Agreement); Form 6 (Affirmation of Receipt of State Ethics Laws Summary); and Form 7 (Iran Certification); Commission on Human Rights & Opportunities’ Nondiscrimination Certification and Workplace Analysis Affirmative Action Report / Employment Information Forms. These documents are included in your BizNet account as fillable forms. Copies of these completed forms may be submitted with your proposal. Any proposer that does not make the certifications required may be disqualified. Respondents must deliver a CD and four (4) paper copies of their complete proposals including the confidential cost proposals to the Department of Motor Vehicles, Emissions Unit Room 115A, 60 State Street, Wethersfield CT, 06161, Attn: John Getsie, by Tuesday, March 10, 2020 2:00 p.m.
- As a point of clarification, all questions should be submitted to John Getsie at john.getsie@ct.gov by Friday, February 14th. All questions and answers will be posted as an addendum.

| Additional Description |
The purpose of this RFP is to seek qualified emissions testing Vendors for the Connecticut Vehicle Inspection Program (CTVIP). The selected Respondent will provide a decentralized statewide enhanced motor vehicle emissions inspection program for Connecticut. The Respondent's RFP will accommodate all required inspection procedures and conform to the Governor's mission to be a digital front door, promoting one stop on-line convenience for customers and less time waiting in line for services. |
