  • The Katharine Hepburn Portico Restoration Project Addendum No. 2 - November 19, 2018 Additional Information Item 1. A Hazardous Materials report is available from Eagle Environmental, Inc.. High levels of lead were detected in the following: • Wood columns and column bases • Concrete patches as column bases • Concrete stair treads and risers • Stucco parging on brick and concrete block walls - Eagle Environmental Report Pg 4 Contractor to include in their bid: 1. Submit two weeks prior to starting Work a written plan, including drawings and diagrams, of proposed materials and methods of protection for treating and removing lead-containing materials, and for preventing harm, damage, and deterioration caused by the Work on the areas identified above to persons (whether involved in the Work or not), building elements, materials and finishes, surrounding landscape and site, and the environment (including air and water). Include procedures for controlling noise and dust. 2. When preparing surfaces for the identified Work, remove all hazardous materials in accordance with the following: a. All applicable Federal, State, and Local laws, codes, and regulations. b. OSHA standards and regulations c. Provide adequate ventilation and dust control to workers and public d. Use proper personal protective equipment e. Properly dispose of stripped paint debris and scraps f. Do not eat near the work area g. Provide 2-day notice to Owner and current occupants when hazardous material removal is to be completed.

  • The Katharine Hepburn Portico Restoration Project Addendum No. 1 – November 11, 2018 Submitted Questions Question 1. On Drawing S1.1 in the notes regarding the wood base, Item 4 calls for all wood to be repainted. We are not sure the extent of the existing wood that would be repainted under this contract. Answer 1. Only the new wood should be primed and painted per Wood Restoration General Notes on S3.1. Question 2. On Drawing S1.2 it states “Peeling Surface Sealant – surfaces to be power washed to remove existing sealant prior to sounding, repairs, and application of new sealant.” There are no “sealants” listed in the specifications. Answer 2. Power-washing requirement of upper base is removed from project. Jahn repair mortar to be texture- and color-matched to existing coating per Cathedral Stone Representative’s training procedures on site. Additional Information Item 1. Report from the Town of Old Saybrook’s Fire Marshal regarding temporary closure of the Katharine Hepburn Cultural Arts Center’s emergency exit. Item 2. Hazardous Materials testing has been performed on the site. The paint at the wood columns was determined to be lead-containing. A report is forth-coming and will be released. Specification Section 030130 Rehabilitation of Cast-in-Place Concrete: Revised for repair of Cast in Place Concrete with Sika System. -------------------------------------------------------------- Town of Old Saybrook Office of the Fire Marshal 302 Main Street (860) 395-3133 First Selectman Mr. Carl Fortuna Town of Old Saybrook 302 Main Street Old Saybrook CT.06475 October 19, 2018 Mr. Carl Fortuna; On October 19, 2018, as per you request I conducted an on scene fire safety assessment of the Kate Theater with Old Saybrook Building Inspector Tom Makowicki located at 300 Main Street in Old Saybrook Connecticut. The purpose of the visit was to examine the front entry way doors which enter the second floor theater from the Main Street level and to determine if they could be safely closed off for thirty days of construction. The answer to this is yes they can be closed for a month during construction with the following conditions put into place. 1. That the closures occur for only the month duration or whatever thirty days are selected. 2. That the front Exit door be completely closed off so as it not represent a means of egress from the Theater. 3. That signage with the words Exit shall be placed to the front access stairs located to the left of the doors being closed off. 4. Temporary Exit signs must be placed at both fronts of the stage doors indicating the other means of egress from the theater. 5. That an announcement, which is already a code requirement, must be made at the beginning of each performance indicating the accessible exits. These simple mandates should adequately keep the patrons moving in a safe and orderly manner in the event of an emergency. These remedies are easily provided with minimal costs. Anything more than thirty days of construction will need to be evaluated at that time. Additional Description Sealed bids for furnishing all labor, equipment and materials as described in the Project Manual (Contract Documents) for the Katharine Hepburn Cultural Arts Center (“The Kate”) Portico Restoration Project in Old Saybrook, Connecticut will be received by the Town of Old Saybrook (Attention Ms. Amy Jagaczewski at 130 Elm Street, Old Saybrook, CT), until 2 p.m., prevailing time, on February 19, 2019. Bidders must be qualified for Work, as determined by the OWNER and SHPO. A mandatory pre-bid meeting and walk-through will be held at 2:00 p.m. on January 29, 2019, at the site. This project is being funded solely by the Town of Old Saybrook. State of CT Prevailing Wage Rates are applicable on this project. As of January 14, 2019, a complete set of Contract Documents (i.e. Project Manual and Contract Drawings) can be obtained on the State of Connecticut portal and on the Town of Old Saybrook website; in addition a complete hard copy of the Contract Documents will be available at the site for review. All work must meet the Secretary of the Interior’s Standards for the Treatment of Historic Properties and be approved by the State Historic Preservation Office (SHPO).
