| Additional Description |
The Community & Clinical Integration Program (CCIP) is comprised of evidence-based care delivery standards that participants will receive support to achieve over a 15-month performance period. CCIP promotes care delivery transformation across accountable physician networks to deliver better care that results in better health outcomes at lower costs for Medicare, Medicaid, and commercial plan enrollees.
CCIP Wave 2 Transformation Awards are only available to entities that are participating in PCMH+ for the performance period starting in May 2018 and who have met the eligibility requirements to participate in PCMH+ Track 1 as outlined in the PCMH+ program. Through this Request for Applications (RFA), qualifying PCMH+ PEs may request initial CCIP Transformation Awards of up $750,000 to enable achievement of the standards. The OHS intends to release a supplemental award opportunity later in calendar year 2018.
